DGMS Forms PDF Download: Application Forms, Certificate Formats and Official Links || Indian Minerology

If you have ever applied for a DGMS certificate, you already know the stress. One wrong format, one old form, one experience certificate that was not on the latest proforma, and the whole application can come back rejected. Most of the confusion starts with one simple thing: finding the right form in the first place.

So I have put all the important DGMS form and format PDFs in one place. Every link below goes to a file on the official DGMS website, so you are not downloading anything from a random third-party site.

Quick note before you download: DGMS has said that applications and supporting certificates not in its prescribed format are rejected. This has been the rule since 01.09.2019. Always check the date and version on the form, and cross-check with the latest notification on dgms.gov.in before you submit.

1. Application Forms

The form you need depends on the certificate you are applying for. These are the direct PDF links I could confirm on the DGMS website.

Gas Testing Application Form Gas Testing Certificate of Competency (Mines Act, 1952) Download PDF
Mate / Blaster Application Form (MMR 1961) Mate's and Blaster's competency exams, Restricted and Unrestricted Download PDF

For the Overman, Surveyor, Foreman and Mining Sirdar application forms, DGMS keeps separate versions for the Coal Mines Regulations, 2017 and the Metalliferous Mines Regulations, 1961. These get updated from time to time, so I am pointing you to the official page instead of a file link that may break: DGMS Exam Forms/Formats/Notices page.

2. Certificate Formats (Medical, Character, Experience)

This is where most applications go wrong. DGMS wants these documents only in its own prescribed format. A certificate on a plain letterhead, even a genuine one, is not accepted.

Things to watch out for

  • Experience certificate: Only the Manager, Agent or Owner of the mine can issue it. The new proforma also asks for the mine's Labour Identification Number (LIN), so do not leave any field blank.
  • Medical certificate: It must carry the doctor's name, signature, registration number, seal and date of issue. A rubber-stamped or facsimile signature can get it rejected.
  • Character certificate: It also needs the issuing officer's name, signature and seal, and it has to be on the DGMS format.
  • Gas Testing candidates: If you are preparing for this exam, you may also like my notes on Gas Testing exam preparation and mine gases.

3. Instructions and Document Checklists

Before you fill any form, read the instructions. They tell you which documents to attach and which ones are only accepted as self-attested copies.

4. Forms Under the Coal Mines Regulations, 2017

These are not exam forms. They are the statutory forms a mine uses for day-to-day compliance, and they come up often in Overman and Manager level papers. DGMS has put all of them in one PDF, including:

  • Form 1-A, 1-B, 1-C and 1-D: notice of opening, reopening, closure and discontinuance
  • Form 2-A to 2-E: notice of change in name, ownership, address, and appointment or termination of agent and manager
  • Form 3: annual returns
  • Form 4-A, 4-B and 4-C: notice of accident or dangerous occurrence, particulars of injured persons, and return to duty
  • Form 5: notice of disease under Section 25 of the Mines Act
  • Form 6: pointing out contraventions during inspections
  • Form 7: Manager's charge report

5. Approval of Mine Safety Equipment

If you work with a manufacturer or supplier, this one is for you. It is the application form for getting safety equipment, materials or appliances approved by DGMS for use in mines. It has to be submitted in duplicate.

How the Application Process Works

  1. Register online. For the computer based tests (CBT), applications for Manager, Overman, Foreman, Surveyor, Sirdar, Mate, Blaster and Gas Testing go through the online link on the DGMS website. No other mode is accepted for the CBT.
  2. Pay the fee through BharatKosh, the government payment portal.
  3. Prepare your documents in the prescribed formats from the sections above.
  4. Oral exams for Mate, Blaster and Gas Testing: in past notifications, candidates had to send the filled proforma to the Secretary, Board of Mining Examination, DGMS, Dhanbad-826001. Read the latest notification for the exact process this year.
  5. After the result, provisionally successful candidates send self-attested copies of documents for verification, and certificates are issued after the originals are verified.

Common Mistakes That Lead to Rejection

  • Using an old version of the form or an experience certificate in a custom format
  • Leaving fields blank in the experience certificate or medical certificate
  • Attaching mark lists or provisional certificates when DGMS asks for a proper pass certificate that shows the date of birth
  • Missing the document deadline after paying the fee. DGMS guidelines have set a time limit, so keep the dates in mind and check the current notification

Frequently Asked Questions

Where is the official DGMS website?

It is www.dgms.gov.in. The forms are under the Examination section, in the Exam Forms/Formats menu.

Can I use my old form if it was valid last year?

Not safely. DGMS has said that forms and supporting documents must be in the prescribed format, so download a fresh copy every time you apply.

Do I need a Gas Testing certificate before other exams?

For most statutory competency certificates, yes. A provisional Gas Testing Certificate can be used to apply for other certificates such as Overman and Foreman. Details are in the instructions PDF linked in section 3.

Is the exam fee paid on the DGMS site?

No. DGMS notifications say the fee is payable only through the BharatKosh website.
